Self -Directed Support Grants
The Board for People with Developmental Disabilities (BPDD) is looking for 10 people to be trained as Self -Directed Support (SDS) facilitators. Facilitators will assist individuals seeking Self- Directed Supports through a Managed Care Organization, Partnership Program or IRIS (Self Directed Supports Waiver) to receive appropriate services. SDS Facilitators will be chosen from different areas of the state in which Family Care has been implemented or is about to be implemented. Consideration will be given to individuals who live in counties where Family Care is currently available or implementation is scheduled for 2008.
In the context of disability services, Self-Directed Supports refers to people's right to control their individual service budgets, to choose where they live and work and to use services and supports outside of the traditional system.
What is the goal of the grants? To assist people with disabilities and their families to understand, access, and utilize Self Directed Support options in their respective county.
What is the role of the SDS Facilitator? To meet with the individual and family, if appropriate, and assist them to navigate through the Family Care system. The SDS facilitator will:
- Recruit at least five individuals in their respective county interested in Self-Directed Supports (SDS) through Family Care or through IRIS
- Assist consumers to formulate and clarify questions regarding Family Care
- Educate consumers about Family Care
- Clarify the roles of the Aging and Disability Resource Centers (ADRC), Managed Care Organization (MCO), Partnership Programs and IRIS Independent Consultants
- Assist with follow up
- Accompany consumers to meetings
- Assist consumers in SDS self-advocacy
- Evaluate how the advocacy went
What are the qualifications of the SDS Facilitator?
- Have access to consumers
- Be able to attend an intensive training on Family Care and IRIS
- Be able to travel within their county
- Have flexibility and availability to meet with consumers, ARDC’s, and MCO’s
- Live in the county where Family Care is active
How much money is available? Each SDS mentor will receive a $2000 stipend.
What is the grant period? September 15, 2008 –up until March 15, 2009. The grant period will end according to consumer needs.
